NDIS Support for Dementia in Melbourne

Nurse-led support for participants with young-onset dementia — medication management, personal care, daily routines, and safety monitoring. Available under NDIS and as a private service.

Understanding Dementia

Young-onset dementia refers to dementia diagnosed before the age of 65. It is a progressive condition that affects memory, thinking, behaviour, and the ability to carry out everyday tasks. Over time, it can significantly impact a person's independence, safety, and quality of life.

For families, the journey can be challenging and emotional. As the condition progresses, the need for clinical oversight becomes increasingly important — particularly around medication management, fall prevention, and maintaining familiar routines that provide comfort and stability.

Evia Health supports NDIS participants aged 17 to 65 with young-onset dementia, as well as participants over 65 who are already on the NDIS. For people over 65 who are not NDIS participants, we also offer private dementia care services. Whatever the pathway, our nurse-led approach means your loved one receives care that is clinically informed, consistent, and respectful.

How We Help

Nurse-led support tailored to the needs of people living with dementia

Medication Management

Managing complex medication regimes with clinical oversight — including monitoring for side effects, interactions, and changes in response over time. Our registered nurses ensure medications are administered safely and on schedule.

Personal Care & Daily Routines

Supporting daily living activities with familiar structure and consistency — showering, dressing, meal preparation, and household tasks. We prioritise dignity, comfort, and maintaining the routines that help your loved one feel safe and oriented.

Safety Monitoring

Proactive support to reduce risks associated with dementia — including fall prevention, wandering risk management, and environmental safety. Our team is trained to identify changes in behaviour or health that may signal a need for adjusted care.

Family Support & Communication

Regular updates, transparent communication, and involvement in care planning. We understand that families are an essential part of the care team, and we work closely with you to ensure your loved one's needs and preferences are understood and respected.

How does nurse-led care help with dementia?

Nurse-led care means your support team works under the clinical oversight of a registered nurse. This is especially important for dementia, where medication regimes can be complex, health needs change over time, and early identification of decline can make a real difference. Our nurses monitor changes, coordinate with GPs and specialists, and ensure care is safe, consistent, and responsive.
