About This Support

What is support coordination?

Support coordination is an NDIS-funded Capacity Building support that helps you implement your plan and get the most from your funding. Whether you need help getting started, changing providers, finding therapies, or building more confidence in the community, your coordinator works with you in a way that is respectful, clear, and focused on what you want — not just what is available. We offer both Level 1 (Support Connection) and Level 2 (Coordination of Supports) to match the level of help you need.

"Before Evia Health, I didn't even understand what was in my NDIS plan. My coordinator helped me set everything up and now I actually feel in control."

How we support you

Plan Understanding & Budget Management

We help you understand your NDIS plan, goals, and funding in plain language — what's funded, how much you have, and how to use it effectively.

Provider Research & Connections

We find and connect you with supports that suit your needs, preferences, and culture — then help you make informed choices about who delivers your supports.

Service Agreement Setup

We review service agreements before you sign, making sure the terms are fair, the pricing is correct, and everything aligns with your NDIS plan and goals.

Plan Review Preparation

When your plan review comes around, we help you gather evidence, document what's working, identify gaps, and prepare so you get the funding you need for the next period.

Crisis & Issue Resolution

When something goes wrong — a service isn't working or an urgent need arises — we step in to resolve it quickly and find alternatives.

Common Questions

Frequently Asked Questions

What is support coordination under the NDIS?

Support coordination is an NDIS-funded Capacity Building support that helps you implement your plan. Your coordinator helps you understand your funding, find and connect with providers, set up service agreements, resolve issues, and prepare for plan reviews — so you get the most from your NDIS plan.

What is the difference between Level 1 and Level 2 support coordination?

Level 1 (Support Connection) helps you understand your plan and connect with providers and community services. Level 2 (Coordination of Supports) is more hands-on — your coordinator works with you to manage multiple providers, resolve barriers, handle crises, and build your capacity to coordinate your own supports over time.

Where does Evia Health provide support coordination?
