Community Nursing Care is an NDIS-funded support that brings registered and enrolled nurses to your home or community setting. They provide clinical services such as health monitoring, medication management, wound care, catheter care, and care planning — so you can receive professional nursing attention without needing to visit a hospital or clinic.
Our nurses can provide a wide range of clinical services at home, including health monitoring and assessments, medication administration, wound care and dressing changes, respiratory support, catheter and stoma care, care planning, and coordination with GPs and specialists.
Community Nursing Care is typically funded under your NDIS plan's Core Supports or Capacity Building budgets, depending on the nature of the nursing services. Evia Health accepts all NDIS funding types — self-managed, plan-managed, and NDIA-managed — as well as private pay.
Evia Health is a registered NDIS provider with a nurse-led model of care. Our Registered Nurses oversee every aspect of your clinical support, ensuring your care plan is followed, your health is monitored, and any changes are picked up early. We are available 24/7 across Melbourne's Bayside and South-East suburbs.
